Registered Nurse (Care Coordinator)

This position functions as a full-time staff Registered Nurse Care Coordinator throughout the Primary Care Service Line. As a Registered Nurse Care Coordinator care will include management of multiple types of psychiatric/behavioral and medical/physical conditions provided in an outpatient setting. Other duties include, but are not limited to:
The selectee will be responsible and accountable for coordinating patient care for individual and groups of patients ranging from the young adult, age 18, to the elderly adult, age 65 or older. The selectee functions as a critical member and leader within an integrated multidisciplinary team. The selectee will be responsible for triage, preventive health screening, immunizations, medication management, crisis intervention, medical/physical and psychosocial assessments. The selectee will provide patient and family education including health promotion and disease prevention; actively participates in quality improvement activities, utilization review and performance measures. The selectee will observe patient behaviors and reaction patterns to modify treatment plans and medications for mental health veterans. The selectee will be expected to work independently and make appropriate referrals to the community or other professional resources as needed. The position is largely providing community based clinical services. He/she must have a valid driver's license; must be physically energetic; mobile and observant. The selectee must be psychologically resilient, resourceful and capable of equanimity and flexibility in the face of recurrent, urgent, stressful interpersonal and group situations. The RN will provide critical clinical support along with all other nursing and technical duties required in the clinic. Work Schedule:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Experience:
Two (2) years of successful general medical-surgical practice experience preferred Two (2) years of Ambulatory Care/Case Manager preferred BSN or higher degree preferred Current BCLS certification Excellent interpersonal and communication skills. Demonstrated ability to assess and prioritize multiple medical and surgical diagnoses. Demonstrated excellent physical assessment skills. Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Functional requirements include light lifting under 15 lbs, light carrying 15-44 lbs, walking and standing and standing for up to 8 hours, use of rapid mental and muscular coordination simultaneously, ability to read type written characters (glasses permitted), ability to hear the whispered voice (hearing aid permitted), repeated bending for up to 8 hours, etc.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
